TRAVEL ASSISTANCE PROGRAM (TAP)
Physicians can order TAP forms for contacting HIBC Provider Services or
by using the TAP automated application service.
Beneficiaries should telephone TAP before making travel arrangements to verify
eligibility and receive a confirmation number. Applicants who present forms without
confirmation numbers will be refused discounts. Applicants can complete their forms
by using the TAP automated application service.
